My Health Passport

Health Passports provide people with arthritis/rheumatism with a health record to help them make the best use of their consultation time with their doctor.

The Health Passport comes in two parts; the first section provides information that does not change – a health record and selection of questions people might want to ask in their consultation; and the second section is a removable monthly ‘diary’ to log changes to physical and emotional health.

Health Passports were first developed in Belgium by Rheumanet and CLAIR working with doctors.  These and they are now available in French and Dutch.
